If you wish to use the default free fund to consolidate any lost Super, simply click on the button on the left entitled "DEFAULT FUND". This will open a PDF file for you to print, fill out and, sign and fax or post back.

If you wish to select your own fund below, you can go through the online form proces and select a fund from a list or nominate your own fund. You will also be required to print, sign and fax or post back an authority form.
